Investigation Summary

Investigation Nr: 200812675
Event: 06/05/2008
Employee Is Struck by Vehicle and Is Killed.

A crew of employees was towing a trailer with a secured "Porta-Pottie". High winds dislodged the "Porta-Pottie" and it fell into the third lane of the highway. The crew proceeded to the next exit, turned around and came back to pick up the "Porta-Pottie". The crew was wearing reflective vests, emergency strobe lights and four-way flashers operating. One employee was directing traffic and Employee #1 and a coworker dragged the "Porta-Pottie" off the road way and were attempting to load it on to the trailer. A driver of a vehicle driving at a high rate of speed with his head turned in the direction of the rear seat; hit Employee #1 who was loading the Porta-Pottie. The force of the collision severed Employee #1's legs and threw him approximately 300 feet. Employee #1 was killed.
