Investigation Summary
At approximately 10:00 a.m. on June 3, 2008, Employee #1 was operating a lawn mower on a steep slope, when it rolled over into a lake and pinned him under the water. Employee #1 susequently drowned as a result of this accident. Employee #1 was operating the mower on a slope that exceeded the manufacturer's recommendation of 10 degrees. Furthermore, the mower was also operating with the roll over protection system disengaged.
