Investigation Summary
On February 28, 2008, Employee #1 of Dubuque Stamping and Mfg., Inc., was operating an Accurshear model 875010, serial #4785, cutting a small piece of steel. There were gaps in between the existing guards and so Employee #1 was able to get his fingers under the guard and amputated the first digits on two fingers.
