Employee Is Killed after Potential Exposure to Electricity

On February 2, 2009, Employee #1 was working alone, performing maintenance work in an electrical panel which contained 480- and 120-volt electrical components. Employee #1's spouse notified plant security staff that he had not come home. Plant security staff initiated a search at approximately 09:00 p.m. At approximately 09:15 p.m., Employee #1's body was found laying on the floor in the supine position in front of the open 6-2 filler panel on the 4th floor in warehouse Number 3. Plant staff attempted to use an AED and administered CPR to Employee #1. Paramedics arrived at approximately 9:23 p.m., and Employee #1 was pronounced dead at 9:35 p.m. Employee #1 was not wearing voltage-rated gloves and had lacerations on both wrists. There was also a label maker inside the panel. On April 13, 2009, the Coroner determined that the cause of death was consistent with acute heart failure due to cardiomyopathy due to hypertensive cardiac disease. The Coroner's report also noted possible exposure to electrical current under "other significant conditions." It was not known if the equipment had been locked out.
