Electric Shock & Fall - Contact with Energized Parts

An employee was replacing light bulbs and using tie wire to secure lights above a damaged ceiling. The employee was standing on a fiberglass stepladder, approximately 1.5 meters above the floor. While he was wrapping a tie wire, the worker received an electric shock and fell. He was hospitalized with fractures to his right shoulder blade and left arm.
