Investigation Summary
Employee #1, #2, #3 and #4 were conducting repairs on a steel rebar cage that was approximately 105 ft tall by 8.6 ft in diameter, weighing 63,354 pounds. The steel rebar cage was suspended from a crane approximately 30 ft above a shaft that was approximately 110 ft deep and filled with slurry. The four employees were standing on the outside of the horizontal steel bands on the upper section of the steel rebar cage. They had their positioning devices attached to their harnesses, which were secured to the cage, while they were realigning and retying the steel bands with wire. The cage came apart and all the vertical steel members, except the four which were rigged to the crane and the horizontal bands that were beneath the rigging tie-off, tumbled down. Employee's #1, #2, and #3 were pulled down into the slurry. Employee #4 remained suspended in the air attached to his harness. Employee #2 and #3 were pulled from the pit. Employee #2, #,3 and #4 received unspecified injuries. Employee #1 was found dead, approximately 20 ft below the top of the slurry.
