Investigation Summary
On April 11, 2009, Employee #1, of Tepco Contract Glazing Inc., was assisting with the movement of the "L" Buck with glass panels. He was working as a spotter for the rough terrain forklift moving the "L" Buck. While moving the glass panels Employee #1 walked in front of the forklift and was run over, crushed and killed.
