Employee Dies after Being Struck by Tarp Arm

On June 3, 2008, Employee #1 was operating a half round dump truck equipped with a motorized tarp system. Employee #1 was standing on the right side of the truck using a motorized tarp system to rewind the tarp. The right side overhead metal tarp arm broke and struck Employee #1 in the head. Employee #1 suffered critical head injuries and died on June 11, 2008.
