Investigation Summary
On April 10, 2008, an employee was working on a 10 by 12 pitched roof at a residential site. The roof was being covered with metal roofing material. While taking a measurement near the roof's peak the employee fell forward, sliding down a section of metal roofing head first and on his back. The employee fell 21 feet to the ground and was killed.
