Investigation Summary
On April 9, 2008, Employee #1 was tightening a "C" clamp to secure a 1,500 lb open web steel truss to a mobile cart. The truss had been positioned to enable Employee #1 to make some minor welds. The truss fell, striking his head and pinning him between the truss and the cart. Employee #1 was transported to the local airport to await air evacuation, but died at the airport.
