Investigation Summary
On June 30, 2008, an employee climbed the ladder to begin securing loose siding on a house. A coworker was holding the ladder. The ladder was on grass with a slope. The employee reached to the right, the ladder shifted and moved to the left. The coworker holding the ladder was unable to steady it and the employee fell off the ladder onto a hard surface and suffered severe head injuries and died. A lack of ladder training was cited.
