Investigation Summary
On March 18, 2008, Employee #1 was assisting with the offloading of steel coils from a railcar. The railcar had a steel top to protect the coils. The coils were unloaded with a 30-ton bridge crane. During the unloading process, the coils were rotated 180 degrees while on the lifting C-hook. Employee #1 was caught between the coil and railcar top when the coil was being rotated. He sustained severe injuries that included head trauma. Employee #1 was transported to a medical center, where he died ten days later.
