Investigation Summary
At approximately 8:45 a.m. on June 25, 2009, Employee #1 and 10 other coworkers were removing a concrete roof from a three-story building. The employees removed the concrete from the metal pan (sheet), when Employee #1 stepped on the metal sheet falling to the second floor. He was not wearing any fall protection. Emergency medical services were summoned. They arrived and transported Employee #1 to Ben Taub Hospital, where he died one day later.
