Investigation Summary
At approximately 2:00 p.m. on December 18, 2008, Employee # 1 died from a possible chemical exposure. Employee #1 was training a coworker on how to take samples from an 18-wheeler trailer. Employee # 1 released the pressure from the top and then opened the lid when he started to bleed from the nose, slumped forward onto the trailer. The coworker grabbed him so he would not fall and then called for help. The security guard and management helped get him to the ground and then summoned emergency services.
