Investigation Summary
At approximately 12:50 p.m. on July 18, 2008, Employee #1 was welding on a 1,000 gallon bio-diesel tank that was in the back of a box Isuzu NPR truck. The bio-diesel tank contained hydrocarbons. The 1,000 gallon tank was said to have a leak. Employee #1 grounded the piping and, then, started welding. The tank had not been cleared of hydrocarbons nor had the tank been vented. The tank exploded fatally injuring Employee #1.
