Investigation Summary
On July 9, 2008, Employee #1, a transportation supervisor and spotter, and a truck driver were involved in a paving operation. Employee #1 requested a truck driver to pull the dump truck, which was carrying asphalt, to the shoulder, due to a faulty power take off mechanism. While on the shoulder, the driver was trying to engage the power take off mechanism, before it began to operate. The driver checked his mirrors before setting the truck into reverse gear and proceeded to move the truck backward. The driver backed up the length of the truck, when he saw Employee #1's body laying on the ground. Employee #1 was killed.
