Investigation Summary
On April 12, 2008, an employee was welding sheeting to a trench box. The track-hoe that was holding the sheeting in place with the buck slipped moving to the left, striking the employee in the right arm. The employee's right arm was amputated, as a result of the injury. The employee was hospitalized.
