Investigation Summary
Employee #1 was inspecting hydraulic leaks inside the container spreader using a six-foot step ladder. The machine's operator opened the spreader to the 40-ft container. Employee #1 was crushed (caught between) by the machine. When the machine was set to the 20-ft container setting, Employee #1 was released from the spreader and fell to the ground. Employee #1 was killed.
