WELDING STEEL DECKING

ON OCTOBER 7, 1985, AT APPROXIMATELY 8:00 AM, EMPLOYEE #1 WAS MAKING PUDDLE WELDS TO A STEEL DECKED ROOF ALONG THE SOUTH OPEN SIDED EDGE WORKING IN A EAST DIRECTION AT 28 FEET. NO FULL PROTECTION IMPLEMENTED. EMPLOYEE #1 HAD JUST BURNT THROUGH THE STEEL DECKING APPROXIMATELY 2' FEET FROM THE SE EDGE, THE WELD WAS NOT COMPLETE. THE EMPLOYEE FELL OVER THE EAST EDGE OF THE ROOF LANDING ON THE LEFT SIDE OF HIS HEAD AND EVENTUALLY DIED FROM SEVERAL SKULL FRACTURES.
