Employee Is Crushed and Killed by Falling Platform

Employee #1 was installing metal roofing and siding from an elevated 3-ft by 16-ft work platform that was attached to a Lull Model Number 3036 with a 30-ft boom. The machine tipped over. Employee #1 fell and was crushed by platform. Employee #1 was killed.
