Investigation Summary
About 11:45 a.m. on February 11, 2010, Employee #1, a reel operator, was working with a crew on a conveyor belt service removal/replacement operation on the Great Lake transport ship, Edgar B. Speer. The crew had installed a conveyor belt-reeling fixture in the forward hold of the ship to spool the new belt onto the conveyor rolls as the old belt was coming off. Employee #1 was walking across a guide above the moving conveyor where it was winding the old belt. His foot slipped onto the moving conveyor, and he was pulled into the 84 in. moving conveyor belt up to his waist before the belt was stopped. Employee #1 sustained five bone fractures to his legs. He was hospitalized for seven days.
