Investigation Summary
On March 24, 2009, Employee #1 was supervising the demolition of fixtures and structures within a department store building in preparation for its renovation for a new tenant. Before work began, Employee #1 went to the buildings mezzanine to determine what obstacles might exist during demolition. During this evaluation, Employee #1 accidentally stepped into a hole in the floor and fell 7.5 feet to the concrete floor below. Employee #1 was taken to a nearby hospital, where he was hospitalized and treated for multiple unspecified injuries.
