Investigation Summary

Investigation Nr: 202488201
Event: 12/12/2008
Employee's Hip Is Fractured Falling from Bobcat

On November 17, 2008, Employee #1, a trainee, was using a Bobcat loader and was dismounting the vehicle. He stated he was attempting to follow company policy to maintain a three point contact with secure surfaces. He rose from the operator's seat, held onto the handhold, and maintained his footing with his right foot on the floor of the Bobcat. He then swung his left leg out to step on the wheel, as trained. As he shifted more weight to his left foot, his left knee collapsed under his weight, and he was unable to hold onto the handhold. His 285 pound body fell to the concrete floor, landing on his left hip. A coworker verified part of Employee #1's account, but thought his foot missed the wheel. Employee #1 was hospitalized with a fractured hip.
