Investigation Summary
At approximately 8:30 a.m., on December 6, 2008, Employee #1 was seriously injured at the employer's jobsite located at 10611 65th Ave. in Los Molinos CA. Initially the employer was told by the hospital it was not considered an amputation because he did not have any bone loss, only the flesh and nail were affected. The employer learned on December 18, 2008 from the workers comp carrier that the Doctor had taken some bone at the time of treatment. A co-owner notified the District Office of the accident by phone at approximately 1:19 p.m. on December 18, 2008. Employee #1 was splitting firewood logs with a Brave Products Inc. #033799 Log-Splitter when a log got caught on the head of splitting ram. When the ram came back, the stuck log crushed his finger at the control leaver. Employee #1 had been using the log-splitter for approximately 2 days prior to the accident and had been trained how to safely use the machine. The splitter was found to be in excellent, almost new, condition at the time of inspection.
