Investigation Summary
On August 27, 2008, an employee, a paving equipment operator was part of a crew paving the new Folsom Dam Road. Paving operations momentarily stopped to clean up an area and to allow employees to get ice. The employee A left his position in front of asphalt windrow elevator and walked around to get ice for his ice chest that he had positioned on a ledge on the front part of the asphalt windrow elevator. The paving operator remained at the controls of the paver but did not see the employee return to his position. The employee standing by the asphalt windrow elevator controls reached inward towards his ice chest with the purpose of placing the ice into the chest. As he was reaching in, the paver operator began moving the unit forward but stopped when heard the employee yell. A small hard tire on the front of the asphalt windrow elevator had come to rest on top of the employee's left foot. After backing the unit up, the emergency personnel from the Folsom Fire Department arranged transportation for the employee to Mercy Folsom Hospital, where he remained until September 2. As a result of the accident, the employee returned to the hospital on September 22, where doctors amputated his large toe on his left foot.
