Investigation Summary

Investigation Nr: 202487732
Event: 10/07/2008
Employee Dies of Massive Trauma to Head

On October 7, 2008, a forklift operator had been alerted by a coworker that a wooden bin had become jammed inside a bin washer. The operator first tried to unjam the bin on the bin washer control side but then walked around the washing line and walked down a narrow area between a wall and the bin washing equipment. For several minutes, the coworker did not see or hear the operator. She left her work station and looked towards the rear of the bin washer. She than observed her operator on the ground. Personnel from around the area responded to the site as did the Yuba City Fire Department, who arranged for the operator to be air-lifted to Sutter Roseville Medical Center in Roseville. The operator, who had sustained massive trauma to his head, died one day later in the hospital.
