Employee Finger Is Injured in Shredder

At approximately 12:00 p.m. on July 1, 2008, an employee was operating a Martin Yale Industries Inc. Model Number 407 shredder. The employee was attempting to unjam the shredder by reaching through the guard, a flap covering the blades at the back of the machine, when a coworker started the machine, catching the employee's finger. The employee was hospitalized and treated hand laceration. According to the Supervisor, workers were not supposed to unjam the equipment themselves, they were instructed to put their hands up and get a supervisor to help. The supervisor would unplug the machine and make the adjustments necessary. The employer did not have these procedures in writing, only verbal instruction, prior to the accident. Since then the procedures have been documented. The machine was labeled Do not touch, keep hands out.
