Investigation Summary
On February 29, 2008 Employee #1 was inadvertently locked into his workplace which was on the 3rd floor of a factory building. Employee #1, who was both deaf and mute, apparently tried to escape by climbing out a third story window and fell to the sidewalk where he was found by the police. Employee #1 was taken to the hospital where he died. At the time of the event there was no apparent condition of fire or other emergency in progress at this employee's workplace.
