Dump Truck Driver Is Run over at a Construction Site

On February 18, 2009, Employee #1, a 27-year-old male dump truck operator with Triangle Construction Road Building, Inc, and, along with a number of other dump truck operators, was removing debris from a recently cleared area of land. As a dump truck was loaded with debris, the other dump trucks were parked about 125-feet from the loading area. All the parked trucks were parked so that the rear of the trucks faced the loading area. When a truck was loaded, it would then be driven from the loading area, and the next truck to be loaded would reverse until it reached the loading area. The truck that was operated by Employee #1 was parked next to the truck (MAC dump truck) which was going to be loaded next. Employee #1 got out of his dump truck cab. Employee #1 then walked behind the adjacent truck. Employee #1 was in a position where the operators could not see him or that he was behind the truck. When the truck reversed to go to the loading area, it struck and ran over employee #1. The cause of death was from crushing injuries of the head, chest and abdomen. The truck that was reversing had a properly functioning back-up or reverse alarm. However, the operators had not been trained about what to do when they lose sight of an employee walking or standing near their vehicle.
