Employee Dies in Collapsed Trench

On October 31, 2008, Employee #1 was installing storm drain piping within a trench that measured 6 ft wide by 43 ft long by 8.5 ft deep. The site foreman decided to have the trench box removed from the trench; however, Employee #1 continued to work in the trench without the trench box or other cave-in protection. While Employee #1 and a coworker were in the trench, its side collapsed. Employee #1 received crushing injuries and a fatal head injury. The coworker was uninjured. Soil testing and analysis determined that the soil of the trench was a Class B soil.
