Investigation Summary
At approximately 8:35 a.m. on December 11, 2009, Employee #1 was walking from the parking lot to the designated employee entrance at the Tire Sales & Service Center, located along the side of the building. While crossing the road in front of the Tire Sales & Service Center, she was killed when she was struck and run over multiple times by a Budweiser delivery truck (2002 Sterling Acterra, Model Number: M8500). The truck was not equipped with a reverse signal alarm.
