Event: 06/22/2008
Employee Falls from Work Platform, Later Dies

At approximately 12:30 p.m. on June 22, 2008, while working from an elevated work platform, an employee was observed cutting plastic wrap to get at bags of sugar on a wood pallet. Immediately behind the employee was a pallet with three remaining bags of salt. There were no witnesses to the accident. It is believed that the employee fell backwards while pulling on the plastic wrapping material, or stepped back and tripped on the toe board along the walkway. The employee fell approximately 4 feet, 5 inches to the concrete floor below, where the employee was found by coworkers. Coworkers got help immediately. Emergency medical services were called and responded to the scene. The employee was transported to the local regional medical center and sent by life flight to Via Christi Medical Center Head Trauma Unit in Wichita, Kansas. The employee died from her injuries on June 30.
