Investigation Summary
On February 3, 2008, were at a site at which over half of the tilt-up walls had already been set in place and braced. Work was in progress at the front of the building, where two separate panels--an inside panel and an outside panel--were to be set. The inside panels had been set and braced the day before. On the day of the accident, the first outside panel had been set in place and braced, and Employee #1 and the coworker had positioned the second outside panel. The two braces for this panel had been placed through an opening in the inside panel that was behind it, to be secured when the outer panel was in position. Employee #1 and the coworker were each holding an outside panel brace when the inside panel (which was already set and braced in place) began to fall toward them (toward the inside of the building). They dropped their braces and ran to escape from the falling wall. The coworker made it to safety, but Employee #1 was crushed and killed by the falling 54,094 lb wall.
