A STEAM VALVE UNDER PRESSURE EJECTED AND STRUCK THE EMPLOYEE
On November 5th, 2008, a steam valve under pressure ejected and struck Employee #1 in the face. The lock out/ Tag out specific procedure did not address the potential stored steam energy. Employee #1 was hospitalized and had a fracture.
