Investigation Summary
On March 26, 2008, Employee #1 was suspending a kite from the ceiling while utilizing an orchard ladder to gain the needed elevation. The ladder slipped out from under Employee #1, causing the employee to fall approximately six to ten feet. Employee #1 struck his head on the floor, causing a serious concussion injury and hospitalization.
