Investigation Summary
On November 30, 2007, Employee #1 was part of a crew installing a large piece of plate glass in a custom-manufactured window casing. Employee #1 was standing on a second-floor deck frame that was temporarily covered with secured plywood. An offset in the deck frame, to be used for the addition of a stairway, was located to the right of the window. Employee #1 stepped to the right and fell approximately 8 feet to a mound of dirt below. Employee #1 was stabilized at the scene then transported by ambulance to the hospital, where he was admitted overnight and treated for neck and back injuries.
