Rotating machinery fractured hand

On November 8, 2007 Employee # 1 was operating an Okuma LB15 CNC Lathe/Turning Center and the interlocked door to the machine was turned off with the keyed switch. When the shift ended, Employee #1 opened the door to the machine's point of operation and associated rotating and moving internal parts. Employee # 1 reached into the machine to clean out the metal chips below the spinning chuck. The hand was struck by the chuck and resulted in a fracture to some of the right hand's bones.
