Investigation Summary
On January 22, 2008, Employee #1 was on the third tier of a metal scaffold, lowering 2 in. by 12 ft wooden planks to a coworker on the second tier of the scaffold. He had his right hand on the top rail and was using his left hand to lower the planks when the railing and post gave way. Employee #1 fell approximately 24 ft to the ground. He sustained injuries, but did not require hospitalization.
