Investigation Summary
On April 8, 2008, an employee was part of a crew applying stucco and plastering to the outside of a home. At approximately 9:00 a.m., the employee turned off the stucco pump due to a clog in one of the mud hoses. As the employee was bending over to pick up a portion of the hose at a connector and coupling area, a metal shank coupling, installed between and inside the two hoses, split in half. One of the hoses, still under pressure, whipped up into the air and struck the employee in the face and neck. A coworker, standing three feet behind the employee, saw him fall to the ground face down, take two breaths and then lay still. The coworker, with a help from another coworker, turned the employee over at which time he started to breath again. They supported the employee until the ambulance arrived, and he was transport to a nearby medical facility. The employee did not regain consciousness, sustained massive head and neck injuries and died two days later.
