Investigation Summary
At approximately 9:45 a.m. on January 31, 2008, Employee #1 was attempting to take down an extension ladder. A board, which had been placed on the parapet wall to protect the cornice from damage, fell approximately 24 feet and knocked Employee #1 to the ground. The employee was hit on the head by an L-shaped board, weighing approximately 25 pounds, which caused serious head and neck injuries. Employee #1 worked for Stout Roofing, Incorporated.
