Employee Falls from Duct and Is Killed

On January 12, 2008, Employee #1 and a coworker were completing the installation of the pipes. To access the duct work, they used a Skyjack scissor lift. The coworker took the Skyjack scissor lift back to the ground level and went up to the roof. Employee #1 was working approximately 20 feet above ground level installing risers to the water supply and return pipes with a coworker. After completing the connections, the coworker told Employee #1 he was coming to get him. As the coworker was on his way back to the Spa Level, he was told a man was down. Employee #1 had fallen from the duct work and suffered severe head trauma and died on site
