Investigation Summary
On December 22, 2007, Employee #1 was hired by a homeowner to remove shelving units and fabricate and install a French door at a finished private residence. Employee #1 was working alone in the home when he apparently fell from an A-frame ladder, striking his head against the stone tile floor as he landed. Two coworkers arriving at the home to do other work found him lying on the floor, unconscious. Emergency Services responded and transported Employee #1 to University Medical Center, where he died the following day.
