Employee Is Killed When Crushed Under Falling Truck

On February 26, 2008, Employee #1 was repairing a Ford E-450 vehicle mounted with an ambulance unit. He raised the vehicle from the rear using a Gray TNT 550 truck and trailer air lift jack by placing the saddles of the jack on the skid plates beneath the extended bumper. He ignored the warning placarded on the jack and operators manual and did not place jack stands under the vehicle. He then went under the vehicle and it shifted. The jack was ejected, and the vehicle fell on his chest, crushing him. Employee #1 was killed.
