Investigation Summary

Investigation Nr: 200531929
Event: 11/08/2007
Falling logs crush worker

At approximately 2:30pm on November 8, 2007, employee #1 was in the process of moving and sorting walnut logs on location at a saw mill. As part of that process, employee #1 was beginning to unload a logging truck and trailer in the sawmill yard. The truck and trailer were equipped with stakes or vertical stanchions projecting approximately 4 feet above the trailer bed to contain the load. Logs on the trailer and truck were piled several feet higher than the stakes and were secured in place with transport chain(s) and binder(s). After the binder on the trailer load was removed, an approximate 18 feet long by 16 inch diameter log rolled off the trailer striking employee #1 in the head. Employee #2 was in the general area and noted that the log had rolled off the trailer. Upon investigation, employee #2 noted that employee #1 was unresponsive. Emergency crews were dispatched including the Dubuque, Iowa Sheriff's Office and Key West Fire/EMS. Employee #1 was transported to The Finley Hospital in Dubuque, Iowa. Employee #1 was dead on arrival.
