Investigation Summary
On November 5, 2007, an employee was transferring containers from the yard to the bays of the warehouse with an Ottowa yard Jockey tractor for unloading, then taking the empty container and moving it to the storage yard for pick up. Approximately 2:30 p.m., the Dock Manager told the employee to retrieve two containers full of coffee and move them to Bay Number 1 and Bay Number 2. The Dock Manager stated that he saw the first container had been backed into Bay Number 1. At approximately 3:15 p.m., the Dock Manager attempted to contact the employee by radio but the employee did not respond. The Dock Manager went out into the yard and found the employee pinned between a container standing alone and another container hooked up to a tractor. When the Dock manager got into the tractor to pull away from the employee, he noticed the truck still on, and that the brake was off. The employee was killed. The probable cause of death was asphyxiation.
