Investigation Summary
At approximately 1:30 p.m., on December 5, 2008, an employee of Brookside Trucking, Inc., a trucking operator located in Maria Stein, Ohio (SIC 4213; NAICS 484121), received fatal injuries while operating a 2000 Sterling truck with a Multicat stone spreader system (s/n MC21-19, 99M5114) manufactured by Bay-lynx. The employee was performing normal job duties when using the truck's stone spreader conveyor system to deliver stone to the foundation of a new residential construction site located in Findlay, Ohio. After completing the delivery, and with the feed conveyor belt operating in the reverse direction, the employee left the operator control station and, for an unknown reason, was positioned below the moving feed conveyor belt. The employee was found with his left arm caught in an ingoing nip point created by the feed conveyor belt and a snug pulley. Local authorities report that upon arrival the hopper was empty and that the throw conveyor boom had been replaced to the side of the truck, ready for road travel. It appears that the employee's left arm and/or loose clothing was caught in the unguarded nip point and, as the feed conveyor ran in the reverse direction, the employee was pulled between the snug pulley and conveyor, which then pulled his body upward into the conveyor structure resulting in asphyxia. The accident occurred outdoors, there was no precipitation, it does not appear weather was a factor. There were no witnesses to the accident.
