Employee Amputates Finger in Rotary Airlock System

At 5:30 a.m. on March 26, 2008, Employee #1 was accessing the Line #4 rotary airlock system (Model 10x10-SD-254569-B) to remove fines and fibers, which had accumulated on the wire guard of the airlock located at the discharge point. A portion of the wire guard was missing, which allowed Employee #1 to inadvertently reach rotating parts of the rotary airlock, resulting in the amputation of the right index finger at the second knuckle. There was one witness. Employee #1 sought treatment at Fostoria Community Hospital in Fostoria, Ohio.
