Employee Is Injured in Fall on Scaffolding

On October 1, 2009, Employee #1 stepped off an extension ladder and onto a carpenter bracket scaffolding. While trying to lift a 2-foot metal pick by himself, he slipped and fell chest first onto the scaffolding. Employee #1's chest plate was bruised, and he was hospitalized.
