Investigation Summary

Investigation Nr: 200041382
Event: 12/04/2007
Employee Is Killed When Struck by Auger

On December 4, 2007, Employee #1 was taking soil samples using an auger attached to a rig. An auger plate with a handle was used to prevent the auger from moving when not in operation. He turned the auger on with the auger plate still in place. The spinning auger came into contact with the auger plate causing it to spin along with the auger. The handle attached to the auger plate struck his ankle and pulled him into the spinning auger. He was thrown to the side of the rig and lost consciousness. He was hospitalized and died.
