Employee Falls from Aerial Lift, Later Dies

On August 14, 2008, Employee #1 was using an aerial lift to repair a jammed conveyor about 19 ft high. The aerial lift possibly malfunctioned, and the basket dropped about four feet, stopping on a bracket about 15 ft high. A coworker brought a 24-ft extension ladder so that the employee could climb down. When the employee left the basket to get onto the ladder, the basket jerked back up to its original position at 19 ft high. The employee was already outside the basket and fell about 15 ft to the ground, landing on his head. Employee #1 later died from his injuries.
